แนวโน้มการฆ่าตัวตายในประเทศไทย:
แง่มุมทางเพศและช่วงวัย

บทคัดย่อ การศึกษานี้มีวัตถุประสงเพื่อวิเคราะห์แนวโน้มพฤติกรรมการฆ่าตัวตายของคนไทยในช่วง
2 ทศวรรษหลัง โดยเน้นในแง่มุมของความแตกต่างทางเพศและช่วงวัย วิธีการศึกษา
ข้อมูลในการศึกษาได้จากสำนักนโยบายและแผนสาธารณสุข กระทรวงสาธารณสุข
ซึ่งรวบรวมสถิติจากมรณบัตรทั่วประเทศในแต่ละปี วิเคราะห์อัตราการฆ่าตัวตายระหว่างพ.ศ.
2535-2539 โดยจำแนกตามเพศ และช่วงอายุ และวิธีการที่ใช้โดยจำแนกตามเพศ
และเปรียบเทียบพฤติกรรมการตายของประชากรจังหวัดกรุงเทพมหานครกับจังหวัดชัยนาท
ผลการศึกษา ค่าเฉลี่ยอัตราการฆ่าตัวตายเพิ่มขึ้นจาก 6.4 ต่อแสนประชากรในช่วง
10 ปีแรก (พ.ศ. 2520-2529) เป็น 6.7 ต่อแสนประชากรในช่วง 10 ปีหลัง
(พ.ศ. 2530-2539) โดยในทศวรรษหลังเพศชายมีอัตราการฆ่าตัวตายเพิ่มสูงขึ้นกว่าเพศหญิงอย่างชัดเจน
โดยเฉพาะเพศชายในช่วงวัย 20-24 ปี (21.7 ต่อแสนประชากรในพ.ศ. 2539)
ส่วนเพศหญิงพบการฆ่าตัวตายสูงในช่วงอายุ 15-19 ปี (6.6 ต่อแสนประชากรในพ.ศ.
2539) วัยสูงอายุพบการฆ่าตัวตายไม่มาก วิธีการฆ่าตัวตายที่พบบ่อยในเพศชายได้แก่การแขวนคอ
ส่วนในเพศหญิงนิยมกินยาเกินขนาดหรือใช้สารเคมี อัตราการฆ่าตัวตายในวัยหนุ่มสาวของจังหวัดชัยนาทสูงกว่ากรุงเทพมหานครมาก
ในขณะที่การฆ่าตัวตายในผู้สูงอายุพบไม่มาก
สรุปควรมีการศึกษาในกระบวนการวิจัยเชิงคุณภาพต่อไปเพื่อที่จะได้อธิบายลักษณะการเปลี่ยนแปลงนี้ได้ชัดเจนขึ้น

Suicide
trends in Thailand: categorized by age and gender

Objective
To examine suicide trends in Thailand during the past two decades
as categorized by age and gender. Methods Statistics on deaths
in the country is annually collected from the death-certificates
by the Health Information Division, Bureau of Health Policy and
Planning, Ministry of Public health. This data was obtained as the
source of information in this study. In addition to annual reported
of suicide rates and male to female ratio during the past two decades,
suicide statistics during 1992-96 were analyzed by age and gender.
Results The mean suicide rates for 1977-86 and 1987-96 were
6.4 and 6.7 per 100,000 respectively. The male to female ratio increased
from 1.3 in 1984 to 2.9 in 1996 due to a steady decline in female
rates and an increment in male rates. In 1996 the highest rates
occurs in males aged 20 through 24 years (21.7 per 100,000) and
in females aged 15 through 19 years (6.6 per 100,000) while overall
rate of elderly suicides was 10.6 per 100,000. Hanging was the method
most chosen by men whereas women preferred self-intoxicating. Suicide
rates among young adults in Chainat, a small province in the central
region of the country, were rather high compared to those in Bangkok
while elderly suicides were rare. Conclusions More suitable
complementary methods such as an ethnographic study are required
to provided a thorough understanding of these trends.
