AS A SCREEN FOR DEPRESSION IN ADOLESCENTS

ABSTRACT:
Objective To study
the utilization of the CES-D (Center for Epidemiologic Studies
- Depression scale), Thai version, as a screen for depression
in Thai adolescents. Method Subjects which consisted of 125 male
adolescents aged 15-18 years filled out the CES-D and were evaluated
with structured psychiatric interview by psychiatrists who were
blind to the results of the CES-D. Results Subjects with interview
validated depression had significantly elevated CES-D scores (mean
= 25.6, SD=8.8 compared with nondepressed subjects (mean=15.4,
SD=6.7), P<10-6. Furthermore, the CES-D scores increased as
the severity of depression increased. Using the Receiver Operating
Characteristic Curve, a cut-off point of 22 produced the best
overall screening characteristics (sensitivity=72%, specificity=85
% and accuracy=82%) Conclusion The results of this study indicate
that the CES-D efficiently differentiates depressed from nondepressed
adolescents and the optimal cutoff score in Thai adolescents is
higher than the score previously recommended. Since the CES-D
is an economical, easy to administer and readly analyzable instrument,
it should be used as a screen and a supplant for clinical evaluation
of depression in adolescents.
Clinical
study of OCD in children and adolescents.

Abstract :
Objective : The purposes
of this study was to evaluate clinical features of OCD and associated
psychiatric disorders. Method: All pediatric patients with DSM-
III- R diagnosis of OCD who have treated at the Child Mental Health
Center during January 1st,1989 - December 31st, 1995 were studied
from retrospective chart reviews. Clinical features and associated
major psychiatric disorders were evaluated. Results: Twenty -
five identified patients, mean age onset was 11.0+/-2.9 years
and mean of duration of illness before treatment was 1 year. Common
obsessions were contamination,something terrible happening and
religious obsession and common compulsions were ritual repeating,
checking and ritualized hand washing respectively. The diagnoses
frequently comorbid with OCD were depressive disorder (20%) ,
anxiety disorder (20%) and Tics(8%). Conclusion : Although limited
by their retrospective nature , these findings of OCD clinical
features can be used as database for further research.

Diagnostic stability
of functional psychoses

Abstract :
The Objective of this study
was to study diagnostic stability of functional psychoses, pattern
of diagnostic change, and factors related to diagnostic stability.
Methods : Data were obtained from hospital records of all first
admissions in Somdet Chaopraya Hospital between January 1, 1988
to December 31,1988. Only patients who were diagnosed as functional
psychoses according to ICD-9 criteria and were readmitted at least
one time during a five- year follow up period were selected. Diagnostic
stability means measurement of degree to which psychiatric diagnosis
remained unchanged at a later admission. Results : 360 patients
with functional psychoses were studied. High degree of diagnostic
stability were found in the initial diagnoses of schizophrenia
(83.92 %), followed by affective psychoses (57.53%). Low degree
of stability were found in the initial diagnoses of other nonorganic
psychoses (3.57%) and paranoid states (0 %) .The pattern of diagnostic
change were described. Factors related to diagnostic stability
were examined and discussed.

DEPRESSION IN CHILDREN &

Abstract The author has reviewed
the articles related to childhood and adolescent depression and
presented by comparing to the same, more familiar syndrome of
adult. The purpose was to enhance the pictures of this disorder
in a more diversified and deeper dimensions. The greater ability
to detect and provide care to this group would certainly preventing
the social and mental health problems in the years to come.
