บทคัดย่อ
ผู้เขียนได้รวบรวมผลการศึกษาวิจัยเกี่ยวกับ กลไกการทำงานของสมองที่เกี่ยวข้องกับการติดสารเสพย์ติด
โดยหลักฐานจากการศึกษาต่าง ๆ ระบุว่า mesolimbic dopamine system หรือ
brain reward circuit เป็นสมองส่วนที่มีการทำงานเกี่ยวข้องกับ reinforcing
property ของสารเสพย์ติด และการใช้สารเสพย์ติดต่อเนื่องกันเป็นเวลานานจะทำให้เกิดการเปลี่ยนแปลงของการทำงานและโครงสร้างของ
dopamine neuron ในระบบดังกล่าว ซึ่งเชื่อว่ามีความเกี่ยวข้องกับการเกิดพฤติกรรมเสพติด
รวมทั้งยังพบแนวโน้มว่า ความบกพร่องของ gene ที่ควบคุมการทำงานของ
dopamine อาจเป็นปัจจัยส่งเสริมให้เกิดการเสพติดอีกด้วย ซึ่งจำเป็นต้องอาศัยการศึกษาเพิ่มเติมในอนาคตเพื่อยืนยืนหรือหาข้อสรุปที่ชัดเจนต่อไป

Abstract
The author
has reviewed literature on brain mechanisms that are involved in
the substance dependence. Evidences indicate that mesolimbic dopamine
system, also known as brain reward circuit, is responsible for the
reinforcing effect of the drugs of abuse and chronic exposure to
drugs may lead to both functional and structural changes in neurons
of this brain system. These long-term changes are believed to be
related to craving and compulsive use seen in chronic substance
abusers. Also, genetic factor ( especially genes that control dopamine
transmission ) might contribute to vulnerability to drug addiction.
However, definite gene defects and exact mechanisms are still unknown.
Further study needs to be done in the future with the goal to provide
cure to clients.

การติดสารเสพย์ติดเป็นความผิดปกติที่มีความเรื้อรังเนื่องจากผู้ป่วยที่ได้รับการรักษาแล้ว
ส่วนใหญ่จะกลับไปติดใหม่ซ้ำแล้วซ้ำอีก ทำให้เกิดความจำเป็นในการค้นหาวิธีการรักษาที่ได้ผลดีกว่าในปัจจุบันซึ่งการพัฒนาการรักษาให้มีประสิทธิภาพเพิ่มขึ้นในลักษณะที่ทำให้ผู้ป่วยหมดความต้องการใช้สารเสพย์ติด
หรือเลิกเสพอย่างถาวรได้นั้น ต้องอาศัยความรู้ความเข้าใจอย่างละเอียดว่าสารเสพย์ติดที่มีการเสพเข้าสู่ร่างกายนั้นมีกลไกการออกฤทธิ์หรือมีผลต่อการทำงานของสมองอย่างไรจึงเกิดการเสพติดขึ้น
โดยความรู้ดังกล่าวจะเป็นพื้นฐานที่นำไปสู่การช่วยให้ผู้ป่วยกลุ่มนี้เปลี่ยนเป็นประชากรที่มีประสิทธิภาพของชาติในอนาคต
ฤทธิเสพติด
สารที่ทำให้เกิดการเสพติดได้นั้นต้องมีคุณสมบัติเฉพาะที่เรียกว่า
reinforcing property ซึ่งหมายถึง การที่สารดังกล่าวสามารถทำให้ผู้เสพเกิดความต้องการใช้ซ้ำอีก
หรือใช้อย่างต่อเนื่อง การทดสอบคุณสมบัติดังกล่าวของยาหรือสารจะอาศัยการทดลองในสัตว์
(animal self-administration)1 โดยวิธีซึ่งเป็นที่ยอมรับกันคือ
การสอด catheter เข้าไปในเส้นเลือดดำของสัตว์ทดลอง ปลายอีกข้างของ
catheter จะต่อเข้ากับ pump และที่บรรจุสารที่ต้องการศึกษา สัตว์จะถูกฝึกให้เรียนรู้ว่าการกดแป้นหรือสวิทช์จะเป็นการฉีดสารทดลองเข้าสู่ตัวเอง
การที่สัตว์ซึ่งถูกฝึกแล้วเลือกฉีดสารชนิดใดเข้าสู่ตัวเองโดยสมัครใจ
ถือว่าสารดังกล่าวทำให้เกิด self-administration และจะถูกจัดเป็นสารที่ทำให้เกิดการเสพติดได้
ในการอธิบายว่าสารเสพย์ติดทำให้เกิด
self-administration ในสัตว์ หรือการใช้อย่างต่อเนื่องในมนุษย์ได้อย่างไรนั้น
หากพิจารณาในแง่คุณสมบัติทางเภสัชวิทยาของสารเสพย์ติดแต่ละชนิด เช่น
opiates ลดอาการปวดและทำให้เกิดการเปลี่ยนแปลงของอารมณ์ (mood) โดยการกระตุ้น
opioid receptors ในสมอง 2 ในขณะที่ amphetamine กระตุ้นให้มีระดับ
dopamine และ norepinephrine เพิ่มขึ้นในบริเวณ synapse 3
ทำให้เกิดการกระตุ้นอารมณ์ และ ความตื่นตัว (arousal) สำหรับ ethanol
นั้นทำให้มีการเปลี่ยนแปลงใน การทำงานของ GABAA และ NMDA
- glutamate receptor เป็นผลให้เกิดการผ่อนคลายและการเปลี่ยนแปลงของอารมณ์
ในภาวะ intoxication 4, 5 ส่วนฤทธิ์หลอนประสาท (hallucinogenic)
ของ MDMA (ecstasy หรือ ยา E) เป็นผลจากการเพิ่มปริมาณ serotonin
ในสมอง 6 แต่ใน ketamine (ยา K) นั้นเป็นผลจากคุณสมบัติ
NMDA receptor antagonist ใน cortex และ limbic system 7
ซึ่งจะเห็นได้ว่า สารเสพย์ติดชนิดต่าง ๆ มีกลไกการออกฤทธิ์ต่อสมองที่มีลักษณะหลากหลาย
แต่ทุกตัวทำให้เกิดการเสพติดได้เหมือนกัน จากเหตุผลดังกล่าวนี้ทำให้เกิดข้อสันนิษฐานตามมาว่า
สารเสพย์ติดทั้งหลายน่าจะมีลักษณะ ร่วมกันบางประการโดยมีการออกฤทธิ์ต่อสมองหรือการทำงานของสมองส่วนใดส่วนหนึ่ง
โดยที่สมองส่วนดังกล่าวมีการทำงานที่เกี่ยวข้องกับการเสพติดโดยเฉพาะ

ในการทำ
intracranial self stimulation ในสัตว์ทดลองโดยการฝัง electrode ลงไปในสมองของสัตว์และฝึกให้สัตว์ดังกล่าวเรียนรู้ว่าการกดสวิทช์จะเป็นการปล่อยกระแสไฟฟ้าเพื่อกระตุ้นสมองตัวเองนั้น
พบว่ามีสมองบางส่วนเมื่อถูกกระตุ้นด้วยไฟฟ้าแล้วทำให้สัตว์ทดลองเกิดความพอใจและกระตุ้นสมองตัวเองซ้ำ
8 ซึ่งการศึกษาดังกล่าวในมนุษย์โดย Heath ในปี 1964 9
ให้ผลตรงกัน โดยผู้ถูกทดลองบรรยายความรู้สึกว่าการกระตุ้นสมองดังกล่าวทำให้เกิด
euphoria ขึ้น ผลการศึกษาในช่วง 15 ปีต่อมาระบุว่า สมองส่วนที่เกี่ยวข้องกับ
intracranial self stimulation คือ mesolimbic dopamine system ซึ่งประกอบด้วย
nuclei และ neural fibers (projections) ที่เชื่อมระหว่าง brain stem
กับ cortex ได้แก่ ventral tegmental area, nucleus accumbens, prefrontal
cortex ฯลฯ 10, 11, 12 และเรียกสมองส่วนนี้ว่า brain reward
circuit หรือ pleasure centers โดยมี dopamine เป็นกลไกการทำงานที่สำคัญ
13, 14, 15
ในการศึกษาถึงความสัมพันธ์ระหว่าง
brain reward circuit ดังกล่าวกับสารเสพย์ติด พบว่า cocaine, amphetamine,
opiates, barbiturates, benzodiazepine, phencyclidine (PCP), ethanol,
tetrahydrocannabinoid (กัญชา) และ ketamine สามารถทำให้เกิดการเปลี่ยนแปลงพฤติกรรม
intracranial self stimulation ของสัตว์ทดลองทั้งในลักษณะเพิ่มความถี่ของการกระตุ้นสมองตัวเอง
และลด threshold โดยสามารถกระตุ้นสมองด้วยกระแสไฟฟ้าที่ต่ำลงกว่าเดิม
16, 17, 18, 19, 20, 21, 22, 23, 24
นอกจากนี้การศึกษาโดยใช้เทคนิค
micro injection ฉีดสารเสพย์ติดเข้าสู่สมองส่วนต่างๆ ของสัตว์ทดลอง
เพื่อศึกษา intracranial self-administration พบว่าสัตว์ทดลองจะ self-administer
amphetamine เข้าสู่ nucleus accumbens และ prefrontal cortex 25,
26 โดยตรง ในขณะที่สารเสพย์ติดชนิดอื่น เช่น cocaine จะเป็นสมองส่วน
prefrontal cortex 27 ส่วน morphine นั้นสัตว์ทดลองจะ self-administer
เข้าสู่ ventral tegmental area และ nucleus accumbens 28, 29
ซึ่งสมองส่วนต่างๆ เหล่านี้ล้วนอยู่ใน mesolimbic dopamine system
จากหลักฐานข้างต้นทำให้ได้ข้อสรุปว่า
mesolimbic dopamine system หรือ brain reward circuit เป็นส่วนของสมองที่มีหน้าที่เกี่ยวข้องกับ
reinforcing property ของสารเสพย์ติด ซึ่งทำให้ผู้เสพเกิดความพอใจ
หรือภาวะ high และมีความต้องการใช้ซ้ำอีก ซึ่งนำไปสู่การติดสารเสพย์ติดในที่สุด
ในการพยายามอธิบายว่าสารเสพย์ติดทำปฏิกริยาอย่างไรกับ
mesolimbic dopamine system ในสมองนั้น ได้อาศัย brain microdialysis
เป็นเครื่องมือสำคัญ โดยการศึกษาที่ผ่านมาพบว่า morphine สามารถกระตุ้นให้มีปริมาณ
extracellular dopamine เพิ่มขึ้นใน nucleus accumbens โดยกลไกการทำงานดังกล่าวอาศัย
inhibitory interneuron และ opioid receptors (mu และ delta) 30,
31, 32 ในขณะที่ amphetamine และ cocaine ทำให้ extracellular
dopamine ใน nucleus accumbens เพิ่มขึ้นจากการกระตุ้น dopamine neuron
โดยตรง 33, 34, 35, 36, 37 ส่วน ethanol นั้นทำให้ระดับของ
dopamine เพิ่มขึ้นใน nucleus accumbens โดยกลไกการออกฤทธิ์บางส่วนเกี่ยวข้องกับ
5-HT3 receptor 38, 39, 40, 41, 42 สำหรับ nicotine,
tetrahydrocannabinol (กัญชา) และ MDMA (ยา E ) นั้นสามารถกระตุ้นให้
extracellular dopamine มีปริมาณเพิ่มขึ้นใน mesolimbic dopamine system
ได้เช่นกัน 43, 44, 45 โดยกลไกการทำงานยังไม่ทราบแน่ชัด
การเปลี่ยนแปลงของสมองในภาวะเสพติด
การที่สารเสพย์ติดสามารถกระตุ้นให้มี
dopamine transmission เพิ่มขึ้นใน mesolimbic dopamine system และทำให้ผู้เสพรู้สึก
high ตามที่กล่าวข้างต้นนั้น เป็นผลจากการใช้สารเสพย์ติดเฉพาะครั้ง
(single use) แต่พบว่าการใช้สารเสพย์ติดหลายครั้งติดต่อกันจะทำให้เกิดการเปลี่ยนแปลงพฤติกรรมการเสพโดยมีลักษณะของการใช้ต่อเนื่องกันโดยไม่หยุด
และควบคุมตนเองให้เลิกเสพได้ลำบากหรือไม่ได้เลย (compulsive use) ซึ่งจัดว่าเป็นการเสพติด
โดยพบว่าพฤติกรรมดังกล่าวเป็นผลจากการเกิดความต้องการใช้สารเสพย์ติดที่รุนแรงซึ่งเรียกว่า
craving 46 นอกจากนั้นยังพบว่าในผู้ป่วยที่เลิกเสพได้แล้วเป็นระยะเวลาหนึ่งการใช้สารเสพย์ติดในเวลาต่อมาเพียงครั้งเดียวจะสามารถกระตุ้นพฤติกรรมการเสพอย่างต่อเนื่องที่เกิดในภาวะเสพติดกลับคืนมาได้
ซึ่งลักษณะดังกล่าวร่วมกับหลักฐานสนับสนุนจากการศึกษาในสัตว์ทดลอง
47, 48 ทำให้เชื่อว่าการใช้สารเสพย์ติดต่อเนื่องกันหลาย
ๆ ครั้งจนเกิดการเสพติด จะทำให้เกิดการเปลี่ยนแปลงในสมองที่คงสภาพอยู่เป็นเวลานาน
และเป็นผลให้ผู้ป่วยกลับมาติดใหม่ ( relapse ) ได้ในอนาคต
การศึกษา
conditioned place preference ในสัตว์ทดลอง 49, 50 ร่วมกับ
pharmacological manipulations และ รอยโรค (lesions) ในสมองส่วน mesolimbic
dopamine system ระบุว่า craving ที่เกิดขึ้นในภาวะเสพติดมีความเกี่ยวข้องโดยตรงกับ
mesolimbic dopamine system 51, 52 โดยเฉพาะ
craving ที่เกิดจาก amphetamine นั้น เกี่ยวข้องกับการเปลี่ยนแปลงของ
D1 dopamine receptor 53
นอกจากนี้การศึกษาในสัตว์ทดลองยังพบว่า
การได้รับ cocaine หรือ opiates ติดต่อกันเป็นเวลานานจะทำให้ tyrosine
hydroxylase enzyme เพิ่มขึ้นใน ventral tegmental area และเป็นผลให้
dopamine ที่บริเวณ synapse ใน nucleus accumbens มีปริมาณลดน้อยลง
54, 55 รวมทั้งมีการลดจำนวนของ neurofilament ใน ventral
tegmental area 56 ซึ่งถือว่าเป็นการเปลี่ยนแปลงทั้งโครงสร้างและการทำงานของ
neuron อย่างชัดเจน โดยสิ่งที่เกิดขึ้นดังกล่าวเป็นผลจากการที่สารเสพย์ติดทำให้เกิดการเปลี่ยนแปลงของ
gene expression 57 เช่นในกรณีของ cocaine และ amphetamine
ซึ่งสามารถกระตุ้น c-Fos, c-Jun genes และ activator protein-1 (AP-1)
บน DNA ของ dopamine neuron ใน nucleus accumbens 58, 59
โดยกลไกการทำงานดังกล่าวเกิดขึ้นจากการที่ cocaine และ amphetamine
กระตุ้น dopamine receptor และ cyclic AMP pathway ภายในเซล 60
ซึ่งขบวนการข้างต้นทั้งหมดทำให้มี การเปลี่ยนแปลงของ protein transciption
หรือการสร้างโปรตีนภายในเซล โดยเชื่อว่าการเปลี่ยนแปลงที่เกิดขึ้นเกี่ยวข้องกับการติดสารเสพย์ติด
แต่การเปลี่ยนแปลงดังกล่าวทำให้เกิด craving และ compulsive use ได้อย่างไรนั้นยังไม่สามารถอธิบายได้
ปัจจัยทางพันธุกรรม
การศึกษาของ
Glantz และ Pickens เกี่ยวกับพฤติกรรมการใช้สารเสพย์ติด 61
พบว่าในจำนวนบุคคลที่เคยลองใช้สารเสพย์ติดนั้น มีเพียงส่วนเดียวที่มีการใช้ต่อเนื่องจนเกิดการเสพติดในเวลาต่อมา
ซึ่งพฤติกรรมดังกล่าวเห็นได้ชัดเจนในกรณีของ alcohol dependence จากปรากฎการณ์นี้ทำให้มีการศึกษาเพื่อค้นหาปัจจัยด้านบุคคลที่อาจส่งเสริมให้เกิดการติดสารเสพย์ติด
โดยพบว่าพันธุกรรมเป็นปัจจัยหนึ่งที่มีหลักฐานสนับสนุน เช่นการศึกษา
self-administration ในสัตว์ทดลอง พบว่ามีหนูบางสายพันธุ์ที่ชอบ ethanol
และ self-administer ethanol มากกว่าสายพันธุ์อื่น 62, 63
โดย ventral tegmental area ของหนูสายพันธุ์ดังกล่าวจะมีระดับของ tyrosine
hydroxylase enzyme สูง และมีจำนวนของ neurofilament น้อยกว่าปกติ
64 ซึ่งเป็นลักษณะเดียวกับที่พบในสมองของสัตว์ทดลองพันธุ์อื่น
ๆ ที่ได้รับสารเสพย์ติดติดต่อกันเป็นเวลานาน โดยหลักฐานดังกล่าวยืนยันว่าพันธุกรรมมีบทบาทในการกำหนดพฤติกรรมการใช้สารเสพย์ติด
ในสิ่งมีชีวิต
ในการพยายามค้นหา
gene และกลไกการทำงานที่เกี่ยวข้องกับการส่งเสริมให้เกิดการติดสารเสพย์ติดนั้น
การศึกษาวิจัยจะมุ่งเน้นที่ gene ซึ่งควบคุมการทำงานของ dopamine เนื่องจากเป็นกลไกสำคัญใน
brain reward circuit สำหรับความผิดปกติของ gene ซึ่งควบคุม dopamine
receptor ในผู้ป่วย alcohol dependence นั้นการศึกษาที่ผ่านมายังไม่สามารถสรุปผลได้
65, 66 ในขณะที่การศึกษาในผู้ป่วยกลุ่มซึ่งมีพฤติกรรมเสพติดหรือการติดสารเสพย์ติด
ได้แก่ alcohol dependence, cocaine dependence, compulsive gamblers,
compulsive eaters และ attention-defecit hyperactivity disorder พบว่า
ผู้ป่วยกลุ่มนี้มีลักษณะร่วมกันคือ มีความผิดปกติของ A1 D2
receptor allele ใน dopamine receptor gene 67, 68 ทำให้เกิดสมมุติฐานเกี่ยวกับ
reward deficiency syndrome 69 ซึ่งเชื่อว่าการมี dopamine
reward system ที่ทำงานไม่เต็มที่เป็นปัจจัยส่งเสริมให้มีการใช้สารเสพย์ติดเพื่อกระตุ้นระบบดังกล่าวให้ทำงานเพิ่มขึ้นและทำให้เกิดการติดสารเสพย์ติดในที่สุด
ซึ่งการศึกษาล่าสุดโดย Vanderbergh และคณะ 70 พบว่าผู้ป่วย
polysubstance abusers ส่วนใหญ่จะมี catechol-O-methyl transferase
allele ซึ่งความผิดปกติของ gene ดังกล่าวจะมีผลให้การทำงานของ dopamine
ใน mesolimbic dopamine system ลดน้อยกว่าปกติ ซึ่งเป็นการสนับสนุนสมมุติฐานข้างต้น
สรุป
แม้ว่าในขณะนี้จะมีหลักฐานยืนยันว่า
mesolimbic dopamine system เป็นสมองส่วนที่เกี่ยวข้องกับการติดสารเสพย์ติดอย่างค่อนข้างชัดเจน
แต่ความรู้ที่มีอยู่ในปัจจุบันยังไม่สามารถอธิบายว่า การเปลี่ยนแปลง
ที่เกิดขึ้นในระบบดังกล่าวทำให้เกิดการเสพติดทั้งในแง่ของ craving
และ compulsive use ได้อย่างไร จึงจำเป็นต้องมีการศึกษาเพิ่มเติมในอนาคตเพื่อให้เกิดความเข้าใจถึงกลไกการทำงานของระบบดังกล่าว
รวมถึงการค้นหาความบกพร่องของ gene ที่อาจเป็นปัจจัยส่งเสริมให้มีการตอบสนองต่อสารเสพย์ติดมากกว่าปกติ
ซึ่งการศึกษาวิจัยดังกล่าวจะนำไปสู่การพัฒนาการรักษาในอนาคต เช่นการคิดค้นยา
anti-craving หรือการใช้ gene therapy ฯลฯ เพื่อช่วยให้ผู้ติดสารเสพย์ติดสามารถเลิกเสพได้อย่างถาวร
