ลักษณะทางคลินิกของ Tourette’s disorder ในเด็กไทย

บทคัดย่อ

วัตถุประสงค์             เพื่อศึกษาลักษณะอาการทางคลินิกและโรคที่พบร่วมกับ Tourette’s disorder ในผู้ป่วยเด็กที่มารับการรักษาที่สถาบันสุขภาพเด็กแห่งชาติมหาราชินี

วิธีการศึกษา              ผู้รายงานรวบรวมรายละเอียดของผู้ป่วยใหม่ที่ได้รับการวินิจฉัยจากจิตแพทย์ตาม DSM-IV ว่าเป็นโรค Tourette’s disorder ณ คลินิกจิตเวชเด็กและวัยรุ่น  สถาบันสุขภาพเด็กแห่งชาติมหาราชินี ในช่วง พ.. 2533 – 2543 รวม 11 ปี  จำนวน 43 ราย    

ผลการศึกษา              พบอัตราการเกิดโรค Tourette’s disorder ร้อยละ 26.7 ของผู้ป่วยใหม่ที่วินิจฉัยว่าเป็น tic disorders  พบอัตราการเกิดโรคในเพศชาย : หญิง = 8.6 : 1    ส่วนใหญ่มาพบแพทย์ในช่วงอายุ 8–9 ปี หรือขณะเรียนอยู่ในชั้นประถมปีที่ 2–3  พบประวัติการเกิดโรค tic disorders ในครอบครัวร้อยละ 24.4  ส่วนใหญ่เป็นการกระตุกแบบ simple motor tic พบบ่อยที่หนังตา แก้ม มุมปาก  พบกล้ามเนื้อกระตุกหลายๆ มัดพร้อมกันได้ร้อยละ 24.4    พบโรคสมาธิสั้นร่วมกับ Tourette’s disorder มากที่สุด รองลงไปคือ anxiety disorder, learning disorder และ mood disorder     แต่พบ obsessive-compulsive disorder ได้เพียงร้อยละ 8.9

สรุป       ลักษณะอาการทางคลินิกและโรคที่พบร่วมกับ Tourette’s disorder ไม่แตกต่างจากรายงานอื่น  ยกเว้นในเรื่องที่พบโรคในเพศชายมากกว่า   มีประวัติ tic disorders ในครอบครัว และการเกิด obsessive-compulsive disorder น้อยกว่ารายงานจากต่างประเทศ   วารสารสมาคมจิตแพทย์แห่งประเทศไทย 2544; 46(3): 187-194.

Clinical Characteristics of Tourette’s Disorder in Thai Children

Abstract

Objective  To study the clinical features and disorders associated with Tourette’s disorder in children who were treated at the Queen Sirikit National Institute of Child Health.

Method A retrospective study was conducted to gather detailed information of 43 children who were newly diagnosed, based on DSM-V, with Tourette’s disorder between 1990 and 2000 at the Child Psychiatric Clinic, Queen Sirikit National Institute of Child Health. 

Results It was found that Tourette’s disorder accounted for 26.7 percent of the newly diagnosed child patients with tic disorders.   The ratio between male and female patients were 8.6:1, with most of the patients seeking medical treatment when they were 8-9 years old and studying in Grades 2 or 3.  There was a family history of the disorder in 24.4 percent of the cases.  Facial tics occurring at the eyelids, cheeks, and mouth were the most commonly found symptoms, followed by neck and shoulder tics, while another 24.4 percent of the cases suffered from complex motor contraction.  Attention deficit/hyperactivity disorder was found to be the most common co-morbidity, followed by anxiety disorder, learning disorder, and mood disorder.  Obsessive-compressive disorder was found only in 8.9 percent of the cases.

Conclusions  The clinical characteristics and diseases associated with Tourette’ disorder in Thai child patients were not different from those reported in the western countries.  However, The prevalence of family history of tic disorder and obsessive-compulsive disorder was lower than that found in the western hemisphere. บทนำ

                Tourette’s disorder หมายถึงโรคที่แสดงอาการกระตุกของกล้ามเนื้อหลายๆ มัดพร้อมๆ กันหรือมีการเปล่งเสียงออกมาจากลำคอหรือจากจมูก  อาจเป็นคำที่มีความหมายหรือไม่มีก็ตาม  อาการจะขึ้นๆ ลงๆ     เกิดขึ้นเองทันที เกิดซ้ำๆ รวดเร็ว  และไม่รู้ตัว อยู่นอกเหนือการควบคุมแต่ถ้าผู้ป่วยพยายามตั้งใจควบคุมจะทำได้ในช่วงสั้นๆ   โดยที่อาการจะเป็นเกือบทุกวัน  บางครั้งเว้นระยะห่างออกไปแต่ไม่นานเกิน 3 เดือนก็จะมีอาการกลับมาเป็นใหม่  ลักษณะของความรุนแรงหรือตำแหน่งที่เกิดอาการจะเปลี่ยนแปลงได้ในระยะเวลาที่โรคดำเนินต่อไป  อาการแสดงจะเกิดก่อนอายุ 18 ปีโดยมิได้เกิดจากสารกระตุ้นระบบประสาทหรือโรคทางกายอื่นๆ  เสียงที่เปล่งออกมาอาจไม่มีภาษาซึ่งจะเกิดร่วมกับอาการกระตุกของกล้ามเนื้อหรืออาจเกิดไม่พร้อมกันได้1-3

Tourette’s disorder ถูกจัดเป็น neuropsychiatric disorder ที่สืบทอดทางกรรมพันธุ์1-3 มีระดับneurotransmitters ผิดปกติซึ่งเกิดจากความบกพร่องในการทำงานที่บริเวณ basal ganglia4-8 เริ่มแสดงอาการในวัยเด็ก  ส่งผลทำให้คนใกล้ชิดเกิดความรำคาญหรือเป็นที่ล้อเลียน  เกิดปัญหาในการปรับตัวและเข้าสังคมต่อไป

มีรายงานโรค tic disorders ตั้งแต่ ค.. 1489 ซึ่งเดิมเชื่อกันว่าเกิดจากปีศาจมาเข้าสิงในร่างของผู้ป่วย George Gilles de la Tourette ได้รายงานผู้ป่วย Tourette ไว้เป็นครั้งแรก ณ ประเทศฝรั่งเศส ในปี ค..1885 โดยอธิบายลักษณะที่ผู้ป่วยมีอาการกระตุกของกล้ามเนื้อ เปล่งเสียงคำหยาบคาย (coprolalia) และพูดทวนคำ (echolalia)

ยังไม่เคยมีรายงานอาการทางคลินิกของโรค Tourette’s syndrome ในประเทศไทยมาก่อน ผู้รายงานจึงรวบรวมรายละเอียดของผู้ป่วยที่ได้รับการวินิจฉัยโรคและรวบรวมข้อมูลทางวิชาการที่เกี่ยวข้องกับโรค Tourette’s disorder นำเสนอในรายงานนี้ โดยมีวัตถุประสงค์เพื่อศึกษาลักษณะอาการทางคลินิกและรวบรวมปัญหาที่พบร่วมกับโรค Tourette’s disorder ในผู้ป่วยเด็กที่มารับการรักษาที่สถาบันสุขภาพเด็กแห่งชาติมหาราชินี

 

วัสดุและวิธีการ

                ผู้รายงานได้ศึกษารายงานที่สมบูรณ์ย้อนหลังของผู้ป่วยใหม่ที่ได้รับการวินิจฉัยว่าเป็นโรค Tourette’s disorder ตามเกณฑ์การวินิจฉัยของ DSM-IV     คลินิกจิตเวชเด็กและวัยรุ่น สถาบันสุขภาพเด็กแห่งชาติมหาราชินี  ในช่วงปี พ.. 2533 – 2543 รวม 11 ปี จำนวน 43 รายงาน คิดสถิติเปรียบเทียบการเกิดโรคกับกลุ่มผู้ป่วยใหม่ที่วินิจฉัย tic disorders จำนวน  161 ราย     นำรายงานมาแจกแจงในรายละเอียดโดยวิธีการศึกษาเชิงพรรณา  ศึกษาการกระจายโรคในเรื่อง เพศ อายุ ชั้นเรียน ศาสนา เศรษฐฐานะ การดำเนินโรค Tourette’s disorder ในครอบครัว  ความเจ็บป่วยในช่วงมารดาตั้งครรภ์ ขณะคลอด และความเจ็บป่วยในวัยเด็ก อาการแสดงของโรค และปัญหาที่พบร่วมกับโรค Tourette’s disorder

 

ผลการศึกษา

1.      ข้อมูลทั่วไป

ผู้ป่วยใหม่ที่ได้รับการวินิจฉัยจากจิตแพทย์เด็กและวัยรุ่นว่าเป็น Tourette’s disorder ตามเกณฑ์การวินิจฉัยของสมาคมแพทย์อเมริกัน (Diagnostic and statistical manual of mental disorders. 4th  ed   : DSM-IV )  ณ สถาบันสุขภาพเด็กแห่งชาติมหาราชินี  ในช่วงเวลา พ.. 2533 – 2543 รวม 11 ปี   มีจำนวน  43 รายงานจากคนไข้ใหม่ที่ได้รับการวินิจฉัยว่าเป็น tic disorders รวม 161 ราย คิดเป็นร้อยละ 26.7  นำรายงานเฉพาะ Tourette’s disorder มาศึกษาในรายละเอียดต่อไปนี้

อายุ  ผู้ป่วยมีอายุอยู่ในช่วง 5 ปี ถึง 14 ปี อายุเฉลี่ย (mean) เท่ากับ 9.2 ปี อายุที่พบบ่อยที่สุดคือ 8  ปี  อายุน้อยที่สุด 5 ปี 1 เดือน อายุมากที่สุด 14 ปี 1 เดือน (ตารางที่ 1)

ศาสนา  ผู้ป่วยส่วนใหญ่นับถือศาสนาพุทธ ร้อยละ 96.8

ระดับชั้นเรียน  ส่วนใหญ่เรียนอยู่ในชั้นประถมปีที่ 3 และ 2

ประวัติการเจ็บป่วย  ไม่พบว่ามีประวัติการเจ็บป่วยขณะที่มารดาตั้งครรภ์และขณะคลอดในคนไข้กลุ่มนี้  แต่พบประวัติว่ามีความเจ็บป่วยในช่วงวัยทารกและวัยเด็กที่มีผลกระทบต่อสมอง เช่น ประวัติชัก โรคเยื่อหุ้มสมองอักเสบ เป็นต้น  ร้อยละ 14.0

ลักษณะพื้นอารมณ์  ร้อยละ 78.9 จะเป็นคนใจน้อย  ร้องไห้เก่ง  ไม่ค่อยมั่นใจในตัวเอง

สถานภาพครอบครัว  ร้อยละ 95.3 เป็นครอบครัวที่สมบูรณ์  มีเพียงร้อยละ 4.3 ที่ผู้ป่วยอยู่กับบิดาหรือมารดาคนใดคนหนึ่งจากสาเหตุหม้ายหรือหย่าร้าง 

เศรษฐฐานะ   ครอบครัวส่วนใหญ่มีรายได้อยู่ระหว่าง 5,000 – 9,999 บาทต่อเดือนร้อยละ 32.5  รองลงไปครอบครัวมีรายได้อยู่ระหว่าง 10,000 – 25,000 บาทต่อเดือน ร้อยละ 67.4

อาชีพ  บิดาทำงานรับจ้างรายเดือนและรับราชการ ร้อยละ 50.9 และ 22.6 ตามลำดับ  ในขณะที่มารดาทำอาชีพแม่บ้านและรับจ้างรายวัน ร้อยละ 37.7 และ 28.3 ตามลำดับ

ประวัติโรค tic disorders ในครอบครัว  พบผู้ป่วยทั้งหมด 11 ราย หรือร้อยละ 24.4 ที่มีประวัติโรค tic disorders ในครอบครัว  ไม่มีรายใดที่มีประวัติของ Tourette’s disorder

 

2.      อาการแสดงของโรค

ตำแหน่งของอาการกระตุกของกล้ามเนื้อ  อาการกระตุกของกล้ามเนื้อ  พบมากที่บริเวณใบหน้าร้อยละ 62.2  ที่บริเวณลำคอ และที่ไหล่ แขน มือ พบรองลงมาคือ ร้อยละ 34.3 และ 42.2 ตามลำดับ  การกระตุกที่บริเวณท้องพบได้น้อยที่สุด

                ลักษณะของการกระตุกเป็นแบบ simple motor tics พบได้สูงสุด ร้อยละ 75.6 เช่น อาการกระพริบตา สะบัดคอ ยักไหล่ แสยะหน้า กระตุกที่มุมปาก เป็นต้น ส่วนการกระตุกแบบ complex motor tics พบได้ 11 ราย หรือร้อยละ 24.4 เช่น การกระตุกของกล้ามเนื้อหลายมัดที่บริเวณใบหน้าจนรูปหน้าผิดไปจากเดิม ตีหรือกัดตัวเอง กระโดด สะบัดแขนโดนตัวเองหรือสิ่งของรอบๆ

ตำแหน่งของเสียง  เสียงที่เกิดจากบริเวณลำคอพบได้สูงสุดคือร้อยละ 93.0 โดยเป็นเสียงที่เปล่งออกมาเป็นคำอุทานที่ไม่มีความหมาย  มีเพียง 6 ราย หรือร้อยละ 13.9 ที่เปล่งเสียงออกมาเป็นคำหยาบ (coprolalia)  พบเสียงที่บริเวณจมูกร้อยละ 37.2

เหตุกระตุ้นทำให้อาการเพิ่มขึ้น  พบว่าอารมณ์หวาดกลัว  ตกใจ  เสียใจ  เครียดหรือเหนื่อย  เป็นอารมณ์ที่กระตุ้นให้ผู้ป่วยมีอาการแสดงเพิ่มขึ้นร้อยละ 81.3  และอีกร้อยละ  18.6 ที่ผู้ป่วยมีอาการที่ไม่สัมพันธ์กับอารมณ์หรือเหตุการณ์ใดๆ

Comorbidity (ตารางที่ 2)

โรคสมาธิสั้น (attention deficit/hyperactivity disorder–ADHD) เป็นโรคที่พบร่วมกับ Tourette’s disorder สูงที่สุด คือร้อยละ 40,  learning disabilities พบร้อยละ  13.3 โดยที่ผู้ป่วยทุกรายไม่ได้รับความช่วยเหลือทางการศึกษามาก่อน ส่วนใหญ่มีระดับเชาว์นปัญญาอยู่ในเกณฑ์ปกติยกเว้นร้อยละ 6.7 มีระดับเชาว์นปัญญาอยู่ที่ borderline intellectual function. มี 1 รายที่มี mild mental retardation.  พบ motor clumsiness ได้บ่อยแต่ที่มีเกณฑ์การวินิจฉัยของ DSM-IV เข้าได้กับ developmental coordination disorder เพียงร้อยละ 4.4

mood disorder  พบได้ร้อยละ 13.3  เป็น major depressive disorder ร้อยละ 8.8 dysthymia ร้อยละ 4.4  ไม่พบ bipolar disorders

impulse-control disorder พบ conduct disorders ที่มีอาการเกิน 1 ปี เช่น ก้าวร้าวรุนแรง ขโมย หนีโรงเรียน ร้อยละ 8.9, ต่อต้านกฎเกณฑ์และถอนผม กัดเล็บ พบได้ร้อยละ 6.7, จะพบ emotional outburst ที่คุมพฤติกรรมไม่ได้ขณะโกรธได้บ่อยในกลุ่ม Tourette’s disorder with ADHD (ตารางที่3 )  

anxiety disorders พบได้ร้อยละ 15.6  แบ่งเป็น obsessive-compulsive disorders (OCD) ร้อยละ 8.9,   seperation anxiety ร้อยละ 4.4 และ overanxious disorder ร้อยละ 8.9

วิจารณ์

             การศึกษานี้พบการเกิดโรค tic disorders ในครอบครัวของผู้ป่วย Tourette’s disorder ร้อยละ 24.4 ซึ่งต่างจากรายงานอื่นที่พบการดำเนินโรคภายในครอบครัวได้สูงถึงร้อยละ 50 โดยมีหลักฐานว่าเป็นการสืบทอดแบบ autosomal dominant9-11    ส่วนเพศชาย : หญิง เท่ากับ 8 : 1 ซึ่งต่างจากรายงานอื่นที่พบว่าอยู่ในช่วง 1.67–5 : 1 เท่านั้น3,12-13 อาจเนื่องมาจากสถาบันสุขภาพเด็กแห่งชาติมหาราชินีรับคนไข้ที่ถูกส่งต่อจากกุมารแพทย์  จึงทำให้การกระจายของเพศจึงแตกต่างออกไป  อายุที่พบบ่อยอยู่ในช่วง 6-10 ปีซึ่งไม่แตกต่างจากรายงานอื่น

             อาการกระตุกส่วนใหญ่เป็น simple motor tic พบที่ใบหน้า คิ้ว หนังตา มุมปาก  รองลงไปคือที่บริเวณลำคอ ไหล่ แขน มือ   มีเพียงร้อยละ 24.4 ที่เป็น complex motor tic คือมีกล้ามเนื้อหลายมัดกระตุกพร้อมๆกัน เช่น กระโดด สะบัดแขน ไหล่และลำตัวไปพร้อมๆ กัน   อารมณ์เครียด ตื่นเต้น กลัว ตกใจ จะเป็นตัวกระตุ้นทำให้เกิดอาการได้เพิ่มขึ้นจากรายงานนี้  และ 1 รายที่อาการจะเกิดตามหลังการหัวเราะ  และร้อยละ 18.6 ที่อาการไม่มีความสัมพันธ์กับสภาพแวดล้อมใดๆ   ลักษณะอาการที่แสดงออกนี้ไม่แตกต่างจากรายงานอื่น

             อาการเปล่งเสียงส่วนใหญ่ออกมาจากลำคอ  รองลงไปคือเสียงจากจมูก  พบ coprolalia คือเปล่งเสียงออกมาเป็นคำหยาบคายเพียง 2 รายหรือร้อยละ 4.4 เท่านั้นซึ่งตรงกับรายงานอื่นๆ14

             ความรุนแรงของอาการมิได้เป็นตัวบ่งชี้ถึงความรุนแรงของโรค  แต่โรคที่พบร่วมกับ Tourette’s disorder กลับจะมีผลต่อการพยากรณ์โรคและการรักษามากกว่า15-17  รายงานนี้พบว่าโรคสมาธิสั้นเป็นโรคที่พบร่วมกับ Tourette’s disorder ได้บ่อยที่สุดถึงร้อยละ 40  จึงทำให้พบความก้าวร้าว, poor impulse control, emotional outburst และ conduct disorders  ซึ่งพฤติกรรมดังกล่าวอาจเป็นผลตามหลังโรคสมาธิสั้น16-18   Tourette’s disorder เป็นโรคหนึ่งในกลุ่ม neuropsychiatric disorder ซึ่งส่งผลกระทบต่อการทำงานของระบบประสาท  ทำให้เด็กเรียนหนังสือได้ลำบากอยู่แล้ว  โดยพบว่าจะมีความสามารถในการอ่าน เขียน คำนวณได้ต่ำกว่าเด็กปกติอย่างมีนัยสำคัญทางสถิติโดยที่เด็กอาจไม่มีโรคที่พบร่วมอื่นร่วมด้วย19 ในกลุ่ม Tourette’s disorder ที่พบร่วมกับโรคสมาธิสั้น หรือมีทักษะเสียหายรุนแรงจนเข้าเกณฑ์การวินิจฉัยตาม DSM-IV วินิจฉัยว่าเป็น learning disorders (ร้อยละ 13.3) และ coordination disorder (ร้อยละ 4.4)  ซึ่งจะยิ่งทำให้เด็กมีปัญหาในห้องเรียนและการเข้าสังคมได้สูง

             ลักษณะทั่วไปของผู้ป่วยส่วนใหญ่เป็นคนใจน้อย ร้องไห้เก่ง ไม่ค่อยมั่นใจในตัวเอง  โดยพบภาวะ anxiety disorder ได้ร้อยละ 15.6 แบ่งเป็น seperation anxiety (ร้อยละ 4.4),  overanxious anxiety (ร้อยละ 2.2) และ obsessive-compulsive disorder (ร้อยละ 8.8) ซึ่งแตกต่างจากรายงานอื่นที่พบภาวะ OCD ร่วมกับ Tourette’s disorder ได้ถึงร้อยละ 38 – 5020-25 (ตารางที่ 4) ปัจจุบันมีหลักฐานเชื่อว่ากรรมพันธุ์อาจเป็นพื้นฐานของความสัมพันธ์ระหว่าง Tourette’s disorder กับ OCD26-30  โดยพบการเกิด Tourette’s disorder ในครอบครัวของ OCD ได้บ่อยพอๆ กับพบการเกิดโรค OCD ในครอบครัวของ Tourette’s disorder ได้บ่อยเช่นกัน

 

สรุป

             โดยภาพรวม  ผู้ป่วย Tourette’s disorder มีอาการกระตุกแบบ simple motor tic ที่บริเวณใบหน้าและมีเสียงที่เปล่งออกจากลำคอ  พบการพูดคำหยาบน้อย  ส่วนใหญ่อาการจะเพิ่มมากขึ้นถ้าถูกกระตุ้นโดยอารมณ์เครียด  ตกใจ  และพบร่วมกับโรคสมาธิสั้นได้บ่อยที่สุด
