Abstract:

The objective of this research was to study depressive symptomatology in children by using Children's Depression Inventory (CDI). The comparison was done between subjects with interview-validated depression (N=47) and those without depression (N=92). The result of the study were as follows. Depressed children had higher mean CDI score than the non-depressed (18.5 and 9.3 respectively, P>10-6). There was no significant difference in the mean scores of boys and girls and of different age groups. In the analysis of the response to CDI items, it was found that the most common responses were those concerning low self-esteem and ineffectiveness. The second common responses were symptoms of physical disturbances such as decreased appetite and fatigue. Direct expression of depressive feelings was less common. The subscales analysis revealed the depressed subjects to have the highest mean score in negatve mood subscale and the non-depressed in ineffectiveness subscale. Although some depressive symptoms were found in the non-depressed children, those in the depressed group were statistically more severe and more extensive.

Comparison of the Efficacy between Risperidone and Classical Antipsychotics: A Meta-Analysis of Randomized Controlled Trials in Schizophrenic Patients

Abstract:

The efficacy of antipsychotics in the treatment of schizophrenia has been an issue of interest. In the present meta-analysis, the efficacy of risperidone was compared with that of classical antipsychotics. By the CD-ROM edition of Medline computerized search, 8 studies were included in the meta-analysis. The dosage of risperidone was separated into 4 dose ranges. The PANSS positive, negative, general-psychopathology, and total scores in each dose range were analyzed. From the effect size (d) of each comparison, the weighted mean d (dw) of each dose range of risperidone were presented. The total of 2159 patients with schizophrenia or schizophreniform disorder were included in the present analysis.The results show that, in the dosage of 1-2 mg/day of risperidone, the weighted mean effect size (dw) on PANSS positive scores is lower than -0.20. For the dosage of 4-6 mg/day of risperidone, the weighted effect sizes (dw’s) on PANSS negative, general-psychopathology, and total scores are equal to or higher than 0.20. In conclusion, the efficacy of risperidone in the dosage of 1-2 mg/day is likely to be lower than that of classical antipsychotics in regard to the improvement on positive symptoms. However, risperidone in the dosage of 4-6 mg/day is probably more effective than classical antipsychotics, especially on the improvement of negative and general-psychopathology symptoms. Further studies are still needed to support the provisional results of this present analysis.

The Reliability and Validity of Thai version of Hamilton Rating Scale for Depression (full text in Thai)

Abstract:

The Hamilton Rating Scale for Depression, the most widely used rating scale for depression, still has never been translated into Thai. We developed the Thai version of this scale and tested the validity and reliability in Thai patients. Method We translated the Hamilton Rating Scale for Depression into Thai. The back translation by bilingual psychiatrist was done. After the interrater reliability was satisfied, we used this scale with 50 depressive patients. Global Assessment Scale was used as a gold-standard instrument. Statistical analysis was performed to test the validity and internal consistency of the scale. Results The kappa value of the scale was 0.87. The Spearman's correlation coefficient which indicated the validity was -0.8239 (P <0.0001). The internal consistency was good (standardized Cronbach's alpha coefficient = 0.7380). Results of factor analytic studied revealed that our patients’ depressive syndrome pointed toward anxious-depression mixture. Some symptoms, such as poor insight, hypochondriasis, agitation and guilt feeling, yielded low correlation with the total score. This finding might correlated with cultural factors. Conclusion The result revealed that the Thai version of Hamilton Rating Scale for Depression (Thai HRSD) has satisfied validity and reliability. So this rating scale can be used to measure severity of depression in Thai patients.

Treatment of Tardive dyskinesia with Vitamin E

Abstract:

Tardive dyskinesia is an abnormal involuntary movement caused by long-term use of antipsychotic drugs which is difficult to treat. During these recent years there were a large amount of studies trying to use many kinds of medication to treat this condition. But even now there has been no good responses reported from variety of treatment. Vitamin E is one of the interesting drugs among these studies. This paper has reviewed new hypotheses about the pathology of tardive dyskinesia, the pharmacology of vitamin E and reserch responses of tardive dyskinesia to vitamin E treatment during the past decade.
