Reliability and validity of the Thai version of the General Health Questionaire   (full text in Thai)

Abstract The General Health Questionaire (GHQ, Goldberg 1972) was translected into Thai and administered to a sample of 100 people visiting a mobile primary health care unit in Nongchok district, Bankok. Clinical psychary health care unit in Nongchok district, Bankok. Clinical psychiatric interview by psychiatrists was used as a gold standard. Sensitivity, specificity and area under the reciever operating characteristic curve (ROC), as well as the internal consistencies of items were ealenlated. The result of the study showed that all versions of the Thai GHQ (Thai GHQ-60, Thai GHQ-30, Thai GHQ-28, and Thai GHQ-12) had good reliability and validity, with the range of cronbach 's alpha coefficients from 0.86 to 0.95, and the range of sensitivity and specificity from 78.1 % to 85.3 % and 84.4 % to 89.7 % respectively. In conclusion the Thai versions of General Health Questionaire developed can be used as a self-administrative scseening instrument to detect psychiatric disorder in Thai population.


Development of Thai version of Hospital Anxiety and Depression Scale in cancer patients   (full text in Thai)

Abstract The Hospital Anxiety and Depression Scale (HADS) was translated into Thai and administered to a sample of 60 in-patients with cancer to test the validity and reliability of the Thai version of HADS (Thai HADS). Semistructured clinical psychiatric interview was used as a gold standard. The result of the study showed that the Thai HADS had good reliability and validity for both anxiety and depression sub-scales. At the cut-off point of > 11, which was the best cut-off point, the sensitivity of anxiety and depression sub-scales of Thai HADS were 100% and 85.71% respectively, while the specificity were 86.0% for anxiety and 91.3% for depression. Both sub-scales also showed good internal consistencies with Cronbach's alpha coefficient of 0.8551 for anxiety sub-scale and 0.8259 for depression sub-scale. In conclusion, the study showed that Thai HADS is a reliable and valid instrument for the screening of anxiety and depression in Thai patients, and especially in in-patients with cancer.


Pre-test and Post-test Counselling for HIV Infection in 300 Prostitutes of Chiangrai (full text in Thai)

Abstract The author studied the effects of pre- and post-test counselling for HIV infection prospectively among 300 Chiangrai prostitutes from May 1, 1992 to April 16, 1993. The counsellors are paraprofessionals from the HIV/AIDS Collaboration who had been trained and supervised monthly by a psychiatrist or a clinical psychologist from Chiangrai Regional Hospital. The majority of the responses from 255 cases (85%) who received pre-test counselling were feeling fearful, sad and not feeling anything respectively. For those of 102 infected cases, 80 cases of them received the post-test counselling and the most frequent responses were sad, not feeling anything and acceptance respectively. The review of the monthly-supervision records found insecured feelings as a counsellor and as the one who has to inform the bad news.

Serotonin-dopamine antagonists concepts in psychiatry

Abstract Serotonin-dopamine antagonists (SDAs) are novel antipsychotics which have different pharmacological properties from conventional antipsychotics. SDAs possess potent 5-HT2A receptor and weak D2 receptor antagonism, whereas conventional antipsychotics such as haloperidol blocks D2 receptors with higher affinity than 5-HT2A receptors. From their distinct receptor profiles, SDAs effectively reduced negative symptoms and produced less extrapyramidal side effect than conventional antipsychotics, however they are equally potent to conventional agents in treating positive symptoms. A number of SDAs drugs are being developed which will provide new modalities in treating schizophrenic patients and their clinical efficacy remains an issue to be verified.

Treatment of Mania with Valproate

Abstract Lithium is the “gold standard” drug in the treatment of bipolar disorder for both acute and prophylactic treatment. Unfortunately, not all patients respond to lithium and lithium itself has some limitations. Other drugs are considered in this condition. This article reviews related articles on clinical aspect of valproate in the treatment of bipolar disorder regarding drug efficacy, drug administration, treatment guidelines and common side effects.
